Payment and Insurance
At this time, the only insurance companies I bill directly are PacificSource and Kaiser.
I also work with many clients who have other insurance plans and who request reimbursement for out-of-network services. Not all plans offer this option; if you are uncertain about yours, I recommend you contact your insurance company directly and ask the following questions:
•Does my plan have an out-of-network option for outpatient mental health?
•If it does, is there a separate deductable?
•If no deductable, what percentage of out-of-network costs does my plan cover?
•Is there any pre-authorization required for out-of- network services?
Out of network clients may pay the discounted self-pay rate and then seek reimbursement from their insurance company. I will be happy to provide any documentation of services and payments which may be required.
Fees
All fees or co-pays are due at the time of service. Payment may be made by cash, check, credit card, Vemmo or PayPal. Credit cards will be charged an additional $1.00 per transaction to cover part of the cost of providing this service.
Standard rates apply when fees are billed to insurance or to a client for later payment. Self-pay rates apply when clients pay in full at the time of service, and no billing is required.
•Standard (billable) rate individual session: $210
•Standard (billable) couple or family session: $220
•Standard (billable) initial assessment: $240
•Self-pay individual session: $190
•Self-pay couple or family session: $200
In special situations, exceptions to the fee policy and rates may be negotiated. Please let me know if you have circumstances that create a hardship.
